Applicant’s Consent to Conduct a Police Record Check or Police Vulnerable Sector CheckI, the undersigned consent that a representative of the Public Security Service of the MRC des Collines-de-l'Outaouais, located at 7, Edelweiss Road, La Pêche, QC J0X 3G0, telephone: 819-459-2422, toll free: 1-877-459-2422, verify my impediments, i.e., any convictions or pending charges for an offense or criminal act, as well as the past or present existence of any behavior or misconduct that could reasonably give rise to concerns that I may pose a potential risk to the physical or moral safety of the vulnerable persons with whom I work or with whom I may be called upon to work. The offenses listed in Schedule 2 of the Criminal Records Act are also considered impediments, even if they have been suspended from the criminal record. I also consent to the police representative conducting checks using the databases available to them. If there are any impediments, the results will be communicated to me and I will be invited to report to the police to review them and to allow me to be heard and, if necessary, to have them modified.
I, the undersigned, consent to the Service de police de la MRC des Collines-de-l’Outaouais carrying out the necessary checks, from the databases available to them, allowing them to check my criminal record, that is to say any pending charges relating to an offense or a criminal act against me or any conviction for such an offense or such an act for which I have not obtained a pardon.
